The receptionist at the practice will give you a form to fill in (known as GMS1). Please complete the form, and your new medical card will be sent out to you. You may be asked for proof of your identity, nationality and entitlement to free healthcare, if you are not a British or EU citizen.
A patient no longer registers with an individual GP or doctor, but with the practice. You can see any doctor/GP at the practice.
You have the right to change your practice without giving any formal reason, but is always helpful to speak to the management team first about any complaints or issues you may have. If you move out of the practice boundary area, you will be asked to find a new practice. The boundary area exists to ensure that services can be accessible and provided in a reasonable time for all patients.
Yes.
You may be refused as a patient if you no longer live in the boundary area, and providing you with a safe and effective medical service is impossible. If this is the case, the practice will write to you within 14 days.
Yes.
Any patient who is verbally or physically abusive, uses threatening language or attempts any form of sexual or religous intimidation with any member of staff, will be removed from the practice, and the list. You will recieve a letter within 7 days indicating the suggested course of action and the grounds for it.
If you are sick away from home, you can see a local GP practice but if you are in the area for longer than 14 days, they may ask you to register as a temporary patient.
If you are sick out of hours and cannot wait until the surgery opens, then contact 111.
GP based telephone/Video Triage System will enable your practice to maintain a high standard of infection control and minimise the spread of Corona virus and infections. You as the patients can benefit enormously from this system:
1. You can avoid an unnecessary visit to the surgery
2. You dont need to take time out of work, or make an avoidable journey to the surgery.
3. When you need access to a GP you will be able to speak to a GP if it is thought appropriate.
4. Please give detailed information to the reception staff, include a telephone contact number and indicate if you have access to a smart phone for a video consult.
